Lawyer's Professional Liability Insurance: What information should I gather before applying?The following documents may be needed and can help you complete your application more quickly
- If you currently carry malpractice insurance, please have a copy of your Declarations page and any attached Endorsements
- Letterhead, if used currently or will be used
- Any Disciplinary complaints and resolution documents, if applicable.
- 5-year loss runs, if applicable

Business Insurance: Is there information I should gather before applying?The following information may be required to quote you for different business insurance lines.
- Please be ready with your formal entity name that is registered with their secretary of state’s office
- Your firm's FEIN number
- The year your law firm was established
- An estimate of your anticipated annual revenue and total payroll
- The total number of staff, broken out by:
- Equity partners/ owners
- All other attorneys
- All other staff
- Basic information about your main office location:
- Your firm’s occupied square footage
- The same information on any additional office locations
- An approximation of your Business Personal property replacement cost value if greater than $5,000
- Bonus question: what are their renewal dates for each BI type (is this a requested or actual eff date?)

Commercial Auto Insurance: Is there any additional information I might need?
The following information will be needed during the application process, but not immediately:
- After you submit your interest form, please be prepared to share the name and address of any Additional Insured employees ~ or a copy of your existing policy declarations page.
- If there is a loan on any car, please have the bank information.
