What information should I gather before applying?

The following documents may be needed and can help you complete your application more quickly:

1. If you currently carry malpractice insurance, please have a copy of your Declarations page and any attached Endorsements
2. Letterhead, if used currently or will be used
3. Any Disciplinary complaints and resolution documents, if applicable
4. 5-year loss runs, if applicable
